How strict are assessors on the structure of text responses, especially the intro? Is there a set way that you must do it in order to not lose marks?

I don't think there is a set way. The structure is there to make sure your ideas are laid out in a way which makes sense. But in saying that, you must have an intro, body paragraphs and conclusion.
