How to write a conclusion for a practical report

Can someone share how they write a conclusion for a science prac at school?

Whenever we receive a prac the conclusion part just says what you have learnt but is their a step by step thing you need to cover in your conclusion?  Also how should you end your conclusion?

Here you go, my chem teacher wrote this for us:
Conclusion: Briefly sum up what you have learnt in one sentence. Was your hypothesis supported or refuted?
